Understanding orthotics for the feet

The human foot is an intricate mechanical structure containing 100 muscles, ligaments and tendons, 26 bones, and 33 joints.

We get problems with our feet because nature designed them for natural surfaces like soft soil and sand, not the hard, synthetic surfaces we walk on today.

The longer that foot issues like plantar fasciitis remain unaddressed, the longer the healing process will take. More serious complications may also arise.

Foot orthotics is the medical field concerned with artificial devices called orthoses (or orthotics) that are inserted into footwear to ensure functionality and balance; they also cushion the foot against impact.

Orthotics can be used to treat many foot issues. Although customised orthotics will fit your feet better and are more precise at correcting biomechanical flaws in your feet and legs than ready-made products, they’re also a great deal costlier. Off-the-shelf foot orthotics will suit 80% of the people who need them.

Orthotics for flat feet, bunion, heel pain, plantar fasciitis, and under- and over-pronation

Flat foot orthotics – also called arch orthotics – can benefit people with fallen arches, a condition that causes pain in the inside and middle of the foot. Orthotics for flat feet redistribute pressure to where it should be – towards the outside of the foot.

The issues associated with flat feet can be related to over-pronation, when the foot rolls inwards after heel strike. However, not everyone with fallen arches has over-pronation.

Paediatric orthotics can be beneficial in cases where a child has flat feet or over-pronation.

Flat feet can also result in bunions when the big toe is bent upwards and pulled towards the second toe, causing a lump on the base of the big toe. Bunion orthotics slow the progression of bunions by treating the mechanical problems and instability that caused the condition in the first place.

Orthotics for plantar fasciitis and orthotics for heel pain in general are typically insoles that provide the correct level of arch support to distribute pressure and stabilise the arch.

Under-pronation is not as common as over-pronation and is more complicated – typically the result of several factors. With under-pronation, excessive pressure is put on the outer edge of the feet.

Orthotics for under- and over-pronation slow the rate of pronation to minimize or eliminate issues related to excessive motion.

A further problem where orthotics can help is in cases of metatarsalgia – a non-specific diagnosis for pain in the forefoot. Metatarsal orthotics can provide effective treatment with a metatarsal bar or pad.

Pronation affects many sports enthusiasts, particularly runners and cyclists. Sports orthotics, running orthotics, and cycling orthotics can correct these imbalances by changing the angle of impact of the foot.
